Step 1
I was given a bottle cap with a hole drilled into the top already, so I just put a jump ring in the hole and put the open part up and away from the cap.

I put a layer of glue down first.

Step 3
I then started placing gears and a layer of glue, then more gears and glue. This went on until I was happy with how it looked. I then moved the pendant to where it wouldn't get bumped by myself or the drs or anyone in the hospital. I let it dry for over 24 hours, but if you use anything like the resin that you should use, follow that ones time frame.

If you live in the USA, Michael's craft store has packs of various colored and sized gears for $4-$5 
I assume there are similar products at Hobby Lobby or Joanne's. You could also order them online
