This is where I got my pattern from, but you could draw your own.
Turn t shirt inside out, place pattern on fabric and draw around it
Sew tiny stitches on outline, I used back stitch to make it secure. You could machine it. Remember to leave turning and stuffing gap.
Cut around shape, right side out. stuff and sew completely shut.
Cut muzzle and tummy shape from felt. You could do contrasting colour. Sew this in place. Add buttons for eyes
Using black thread, sew mouth, using either tiny chain stitch or split stitch.
