... don't forget the radiation warning symbols!
Once upon a time, I had a brand new, really comfy gray and black striped t-shirt. It fit really nice and the fabric was soft... I wore it once, and then my puppies tore a hole in the front and back of said shirt.

That wasn't the end of it, though!
I cut off the sleeves and neck, cut off a strip from the tube that was left, and sewed it back on as a fold-over waistband. Then I used some transparent fabric medium and red acrylic paint to stencil on some radiation symbols. Next, I made the gasmask patch with another stencil (my boyfriend makes awesome stencils - he made me the radiation one and the gasmask) and took the shotgun shells from an old Resident Evil t-shirt we had laying around. I used the patches to cover the holes that had been torn in the shirt by the aforementioned puppies... and last but not least, used some embroidery thread to do some big sloppy stitches around both. :D
