Study the pedal. Cut out the felts for basic cube shape. Make a cube by sewing the pieces up.
On top, for the 'tone knobs', use the tiny ring thing from (broken) guitar strings. (It might be a little difficult to cut them off from the guitar strings.)
For the 'step-on switch', any tube-like object will do. cut a tiny hole in the top side. Stabilize it from inside.
Stitch them up. I also added the 'input/output' hole on each side, just by sewing some stitches. The whole thing is very tiny, because it has to go with the size of those'knobs'.
Make a special 'user manual' for it.
