More creative that the standard rectangle
I cut a piece of foam board into a flower shape, added quilt batting and fabric (trimming it down as needed) then added the ribbon (stapled it to the back, I found hot glue didn't work too well) and then, instead of the standard button at ribbon intersections, I made a small paper flower and hot glued that there. You could have sewed it on, but I chose not to.
I'll try to get a better pic of it up.

Foam board-$3
cotton batting- less than a dollar's worth
fabric-$3 for one half yard
ribbon-$1 (I used about 2.5 yards)
flower-a few cents
