Can't see my avatar?

Well when i sign in i can't see my avatar on the top left corner next to my username, and i cant see it on my profile either. But i can see it when i comment on something or when im added as "a person who loves this". thats only when it shows. Whats happening and how can i fix this?

Oh and i cant see it on the side of what i just wrote- its just a black square with a grey guy in there-hahah!
