Tiny Santa Knit

I am confused! When I finish row 7 I end up with the six cast off stitches in the middle and three stitches each end. Then row 8 says to "knit 2 stitches four times and cast off, repeat at the other end. How do I do this and why have I three stitches at each end PLEASE.
