Take a plectrum you don't use anymore.
Make a little hole with a nail piercing drill or something else you cam make a little hole with.

Make a little ring on it and you can add a charm on the same ring.

Take a old necklace or a string and make a clasp on it.
Before you add the second part of the clasp, add your plectrum.
