Pretty self explanitory but here goes:

Take your Shrinky Dink sheet (I used the Frosted Ruff N' Ready sheets here) and trace your design.

There's the logo I used if you want it.

Cut carefully around the outlines, making sure to cut INTO the corners so you don't rip the plastic.

(I had a few tears but that's because I was rushing. lol)

Punch your holes where you want them to be at this stage as well.

Now shrink your Shrinky Dinks whichever way you prefer.

Most do it in a toaster oven or regular old kitchen oven but I like to use an embossing gun.

Once you've got it all shrunk, cover the plastic with a sealant to prevent scratches.

I used Mod Podge. I love that stuff.

Now add a chain and rock the shizz out of it!!
