Collect an assortment of buttons, you shouldn't need too many. Find a thin ribbon, the thinner the better, you will understand why when you start to put the buttons on.

Begin to feed the ribbon through the first button. If it is a button with 2 holes in it, put the ribbon through them both. If it is a button with 4 holes in it, put the ribbon through 2 of the holes. This makes the buttons lie down flat.

Keep putting buttons onto the ribbon until you are happy with how many are on it.
